Only Super Admins can create reference forms so they are accessed via the SUPER ADMIN MENU > SETTINGS > REFERENCE FORMS.

 

Click on NEW REFERENCE FORM from the side menu.

Give your reference form a name and set which level you want.

Then you will finally enter a brief description of the reference form and click on the ADD button.

Now is your chance to enter your questions.  Enter the following details:

-Question Name (this is the actual question you want to ask)

-Question Type (This is the format the referee will answer the question in)

Note: The difference between SELECTION LIST and DROP DOWN LIST, is that selection list will allow the user to select multiple options from the list.  Eg.  If you are asking for the referee to enter their gender you would use drop down list as they can only be one gender, but if you were asking how they know the person so say what the volunteers 3 top strengths were from a list, then you would use a selection list.

If the question is set to ANSWER REQUIRED then they will be forced to answer the question before they can submit the reference form.

After you have entered a question click on the ADD button to save and go to the next question.

Once you have finished, click on the QUESTIONS button on the side menu and you can review all the questions which will appear on the reference form.

All information is automatically saved, so once you are happy with your questions you are ready.
